It's simple! Book online via calendar or call our office @ 615-400-7034. You'll get your appointment confirmation via email. The next day we are open we will send our intake forms to you.

Drop off or mail us a swab with your saliva on it. Place swab in a ziplock bag with your first and last name on the bag. If mailing from our to state ensure at least one week for it to arrive at our office prior to your appointment.
